How to access our services

We provide services to adults living with stroke and with other physical disabilities living in Calderdale.  Anyone can make a referral to us – you, family, friends, local organisations and health professionals.

Our Day service runs on Monday, Tuesday, Thursday and Friday from 10.00 am to 4.00 pm and we have a very varied range of activities including crafts, carpet bowls, kurling, darts and many more.  We serve a two course lunch each day and the cook will consider your personal dietary requirements

Our community support service provides individual support for users to undertake activities in the locality

Initial contact

At the initial contact we will collect basic contact details and answer any questions you may have about the service. We will then follow this up with a face to face contact meeting, usually in your own home though this can be any mutually agreed venue, where we will find out more about you, your likes and dislikes, your expectations of the services we have on offer and establish whether our services are appropriate to meet your needs

Trial day/ session—free

Once you have had your assessment you can then agree a date and time to come for a free trial session.  This is an opportunity for you to come along and see whether you think it will meet your needs without making a commitment at this stage to contract the service.  Wherever possible the member of the team you met in your initial assessment visit will welcome you on your trail day to be sure you have that continuity of contact.

Transport

Wherever possible we encourage service users to maintain their independence with transport plans. You may be sign posted to other organisations such as Community Transport or the Access Bus if these services are able to meet your transport requirements. Heath Stroke Club Care Service also has limited transport available for those service users who need specialist door to door support and are unable to access other forms of transport.

 Services and price schedule

Our range of services are designed so that you can purchase as much or as little service as you require to meet both your personal needs and individual budgets. You may wish to attend more than once a week, weekly, fortnightly or monthly it’s entirely up to you.

We welcome both Direct Payment and Self-funding service users.  We encourage new users to be sure to have a local authority care assessment as this may give qualification for a personal budget that can be used to pay for our services.  We also encourage all new users to check eligibility for disability benefits (if they are not already in receipt) as this will also help to pay for any relevant service provision.

Outreach service (available Monday to Friday)
